Spotlight Subscribe showing up on other categories

Hi,

My Spotlight Subscribe form consistently shows up on random posts on my site, even though I've specified it should only show up under the “Honduras” category. 

I've included a screenshot below of it showing up on this post: https://madisonsfootsteps.com/5-days-in-lisbon/ that is not tagged “Honduras" and shouldn't have it.

  You have Multi Form Density enabled and the Roatan form is selected as the form to be featured in additional in-content placements:

  

   

  Selecting a different form for the Multi Form Settings, then clicking Save Form, or choosing Single Form will replace or remove the Roatan form from appearing in all posts instead of just those in the Honduras category.
